Codman has always recognized that nutrition is a crucial part of overall health, and has a strong nutrition department whose team has worked in Dorchester for decades. Our nutritionists know the challenges patients face when it comes to getting optimal nutrition and are skilled at helping them plan, strategize, and get the best nutrition for their unique needs.

Integrated Care

Integrated nutrition care means that if, during the course of a primary care provider visit, a patient could benefit from nutrition counseling or intervention, an expert nutritionist will come and consult with the patient. Whether that consultation is a one-time visit or starts a series of consults, the team will make sure that patients have what they need to meet their nutrition goals.

Some common reasons that patients seek nutritional counseling include:

  • Weight loss/gain
  • Diabetes management
  • Hypertension management
  • Picky eating
  • Gastrointestinal problems
  • Pregnancy
  • General healthy lifestyle information

Group Visits

Codman’s Nutrition team also coordinates and participates in different group medical visits throughout the Health Center, including Diabasics, which helps patients newly diagnosed with diabetes; Don’t Weight, a group for patients trying to lose weight; and our MindBody Medicine program.

Frequently Asked Questions

Patient statements are issued on a weekly basis for any new patient responsibilities that have been identified within the previous week. For accounts with balances less than $25.00, additional statements are issued every five weeks. For accounts with balances of $25.00 or more, if the patient has not paid the balance by the time the next statement is due, the account is referred to our reminder agency, which then begins a series of letters and calls reminding them that their balance is overdue.  No additional statements are sent out.

Your provider can send an electronic prescription to the pharmacy located on the ground floor of the Health Center. You can pick up your prescription prior to leaving the Health Center. You can also get your prescription called in to an outside pharmacy as long as you bring the phone number with you.

Check with your insurance company to see if your prescriptions are covered by your policy. If they are, you may be restricted to certain pharmacies and you may need to make a co-payment.

Codman Square Health Center is an independent facility managed by a community Board of Directors. This allows our community to make the decisions regarding the services we offer.
