Are you a RANDOLPH PUBLIC SCHOOLS student planning to participate in fall sports? Codman is offering convenient clinics so student-athletes can complete required pre-season physical exams:

Early Summer Clinics

• June 25, 26, 29
• July 1, 2

Late Summer Clinics

• August 19, 21, 24, 26, 28

All clinics are 8 am to 5 pm.

• LOCATION: Codman Square Health Center at Randolph High School, 42 Highland Ave., Randolph. Please use our community entrance on the back of the building where the football field is located. Ring bell at the door upon arrival to be let in.

• WALK-INS ARE ACCEPTED, but we encourage you to register your student(s) for their sports physicals and scan the QR Code to download the History
  form. Students must bring the completed History form, insurance information, and ID (patient and parent/guardian) to their appointment.
