How do I make an appointment?
Call our Call Center (617) 822-8271 between 9am and 5pm Monday to Friday. Please arrive 15 minutes early to your appointment to allow time to fill out paperwork.

What do I bring to my appointment?
-
Your insurance card
-Any questions you want to ask your health or our services
-A list of medications you are currently taking (or bring them with you)
-The name and phone number of your pharmacy (if you do not use the Pharmacy at the Health Center)

Can I come to Codman Square Health Center even if I am not a patient there? 
Yes, as long as you have a referral from your primary care doctor with his/her NPI#, Name and authorized visits.

Who owns the health center? 
Codman Square Health Center is an independent facility managed by a community Board of Directors. This allows our community to make the decisions regarding the services we offer.

Where are you located? 
The Health Center is located at 637 Washington St at the intersection of Talbot and Washington Streets in Codman Square, Dorchester. For directions via car or public transit visit here.

Do you provide parking?
Yes, we have a public parking lot directly in front of the Health Center and there is also plenty of street parking surrounding the Health Center.

What are your hours?
Our hours of operation may vary between departments, but our standard hours can be found here and at the bottom of your screen.

What insurance do you accept?

How do I fill a prescription?
Your provider can send an electronic prescription to the pharmacy located on the ground floor of the health center. You can pick up your prescription prior to leaving the health center. You can also get your prescription called in to an outside pharmacy as long as you bring the phone number with you.
Check with your insurance company to see if your prescriptions are covered by your policy. If they are, you may be restricted to certain pharmacies and you may need to make a co-payment.

How often do the bills come out from Codman to patients? 
Patient statements are issued on a weekly basis for any new patient responsibilities that have been identified within the previous week.  For accounts with balances less than $25.00 additional statements are issued every five weeks.  For accounts with balances of $25.00 or more, if the patient has not paid the balance by the time the next statement is due, the account is referred to our reminder agency, which then begins a series of letters and calls reminding them that their balance is overdue.  No additional statements are sent out.

Does Codman provide a shuttle service from Codman to BMC for specialty services once I am referred? 
Yes, we provide this courtesy service for our patients being referred to BMC for specialty services.

Can I walk into Codman Square Health Center and complete an application for Mass Health when I am not a patient? 
Yes, we will process an application for non-patients on a walk in basis.

Can you make an appointment to complete an application for Mass Health?
Yes. You can call the Call Center (617) 822-8271 and ask to schedule an appointment with Insurance Services.

How long does it take to receive an approval once you apply for Mass Health?
It can take up to 60 days or less.

How do I get a referral from my provider?
Call the Call Center (617) 822-8271 and tell the receptionist you would like to request a referral to a specialist. Leave a number that you can be contacted back at throughout the day. Let them know what type of referral you are requesting. Make sure you have seen your provider within the past year or understand that an appointment may be necessary for the provider to make an assessment prior to initiating the referral.
