Anyone Can Be A Hero, Free Naloxone Training - March 2

You can stop overdose death. There’s a neighbor, friend, or family member who needs your help.

Join us Thursday, March 2 at 6:30-8:30pm in the Black Box Theatre (14 Epping Street) for a FREE TRAINING for on how to respond to an opioid overdose using Naloxone (Narcan). Food will be provided.

This training is being offered in partnership with Victory Programs, Inc.

You are now able to sign up for Codman Square Health Center's first patient app, OCHIN MyChart. MyChart gives you direct access to portions of your electronic health record (EHR) where your doctor stores your health information.

You will be able to see the following anywhere, anytime:

  • Your lab results
  • Appointment information
  • Medications
  • Your bill

MyChart also provides new, easy methods of communication with Codman Square Health Center.

Dorchester Winter Farmers Market 2017

The Dorchester Winter Farmers Market returns to the Great Hall on Saturdays from January 14, 2017 to March 27 from 10:00am to 1:00pm. This year enjoy fresh produce, local seafood, beef & lamb, fresh bread, homemade body care products, hand crafted clothing & jewelry, sweets & baked goods, teas and more! The Dorchester Winter Farmers Market accepts SNAP/EBT and provides up to $10 of free food with Boston Bounty Bucks!


Free Tax Clinics in Dorchester

Free Tax Clinics return to the Wellness & Fitness Center, 450 Washington Street, Dorchester beginning January 21. Tax Clinics will run Mondays & Tuesdays 4:00-8:00pm and Saturdays 9:00am-1:00pm at the Wellness & Fitness Center. Taxpayers who earned less than $54,000 in the previous tax year can have their taxes prepared for free and claim important tax credits such as the EITC.


CSHC Awarded PCMH PRIME Certification

PCMH PRIME

December 5, 2016 - Codman Square Health Center has been certified as Patient-Centered Medical Home (PCMH) PRIME by the Massachusetts Health Policy Commission. The designation as a PCMH PRIME location indicates high level PCMH practices that demonstrate the integration of behavioral health into their primary care.

PCMH PRIME promotes best practices for integrating behavioral health care into a patient-centered primary care setting. The program provides benefits to practices and their patients by:

  • Supporting the use of evidence-based guidelines in treating patients.
  • Helping increase patient access to behavioral health care services. 
  • Recognizing practices that deliver comprehensive care by addressing both physical and behavioral health in the practice setting. 
  • Helping practices identify behavioral health issues before they become acute, resulting in better outcomes and improved patient experience. 

This certification reiterates Codman’s innovation in primary care and the high level of integrated services offered to our patients.


CSHC Launches Same Day PrEP Service

December 1, 2016 - On World AIDS Day, Codman Square Health Center (CSHC) launced a new service, “Same Day PrEP” to fight AIDS by preventing HIV infections. Same Day PrEP provides an HIV-preventive medication, Truvada, for HIV PreExposure Prophylaxis (PrEP), on the same day to people with high risk of becoming infected with HIV.

Codman Square Health Center is the first location in Massachusetts to offer a same day PrEP service, eliminating the delays in scheduling and waiting for lab results that are barriers for some patients to receive this important service.
