Jane Tuitt, Nurse Practitioner

Jane provides medical services to registered Health Center members, including physical exams/sports physicals, assessment and treatment of illness, asthma treatment/management, injury care, and health assessments and prevention counseling. Jane looks forward to working with Dorchester’s student support team. Jane is also on staff at Codman Square Health Center where she sees patients in Adolescent clinic every Wednesday and Friday afternoon after school hours.

Ruth Allukian, School Nurse

Ruth sees students who are feeling ill or have other health needs. She can provide over-the-counter medication (with parental permission) and other medication with a doctor’s written orders. Ruth also manages and updates students’ health records (physical exam, allergy, health issues) and required school health records.

*Child must have active insurance. Most insurance accepted, including MassHealth.
*Staff are able to help uninsured students find affordable insurance through Codman Square Health Center

Either have your child pick up the following forms at the nurses office or print out a copy of the enrollment forms using the links below. Return these forms to Sarita Richards in the nurse’s office and your student will be able to start scheduling appointments!
