Health Insurance Information
As of October 2007, it is the law for Massachusetts residents to have health insurance. Insurance helps you avoid many of the high costs of healthcare so that you don’t get stuck with a bill that you cannot pay. Insurance can also provide benefits such as free or low-cost eyeglasses or gym memberships. Unfortunately, the insurance system can be very confusing. That is why we have Insurance Service Coordinators who can help you.
If you DO NOT have health insurance right now:
- Contact our Insurance Service Coordinators (located across from registration in the main area of the health center) to set up an appointment or call them at (617) 825-9660, ext. 8241.
- Bring proof of identity, proof of residency and all documentation for all sources of income
- Based on your income, you will be enrolled in a health plan that you can afford
If you DO have health insurance right now:
The Codman Square Health Center accepts most major insurance plans, but double check with our Insurance Service Coordinators to confirm that the provider you have chosen is covered by your plan. We accept:
