Sandra joined Codman Square Health Center in 1994 as Chief Operating Officer. In February of 2011, the Board of Directors appointed her as Chief Executive Officer of the Health Center.
She began her career in health care as a Nurse. She worked as a nurse for Mass General Hospital and New England Medical Center. In 1985 she transitioned into an administrative role in the health insurance industry. She worked at Bay State Health Care where she held several positions. She ended her career there as the Vice President of Clinical Services.
In addition to her role as CEO of Codman Square Health Center, she is also co-CEO of Dotwell, a partnership organization that was formed between the Dorchester House Multi-Service Center and Codman Square Health Center. Dotwell provides non-clinical services to patients of both health centers.
In June 2007, the Massachusetts League of Community Health Center honored Sandra with an Outstanding Employee Award. She also holds a Bachelor of Science from Simmons College and RN certification. She served on the Women’s Health Partnership Board. She currently serves on the Dotwell Board of Directors, Neighborhood Health Plan Advisory Committee,BMC HealthNet Board and the Codman Academy Charter School Board of Directors.
Cotterell has been involved in nearly every aspect of running Codman. She plans on continuing the vision of building a foundation of preventive and primary care services that engages consumers and embraces innovation — while never losing sight of our mission to provide access to care. One of her first goals is to ensure that the transition between leadership is seamless and doesn’t disrupt patient services.
Cotterell lives in Mission Hill with her husband and three teenagers. She and her husband, Ian, run a Pop Warner football and cheerleading program in Boston and Brookline.

Patti has over 25 years of experience as a CFO with an extensive background in Community Health Centers, Multi-Hospital Systems, Skilled Nursing and sub-acute facilities. She has experience in the start-up of 7 hospitals and a number of clinics including the process of planning, construction, financing and opening. Patti studied Business Law at Suffolk University, and continued her education at California State University in Finance. She also serves as Treasurer and member on several affordable housing boards. She recently completed the UCLA Executive Health Management Program as well as the Mass. Community Health Leadership Program. She serves as an advisory board member for the Mass. Health Education Facility Administration (HEFA) for the CDE issuing New Market Tax Credits.

Dr. Severin joined Codman Square Health Center in 1995, and has served as its Medical Director since 2004. He has been named Chief Medical Officer in February 2011. Prior to joining the Codman team, he served as a physician and Director of Public Health at Serabu Hospital in Sierra Leone. He has also worked as an Emergency Room doctor in Wisconsin and volunteered at L’Hopital Albert Schweitzez in Deschapelles, Haiti.

Tony focuses his practice on strategic, development and external relations efforts, including government and community relations, marketing, public relations, communications and governance matters. He is currently managing the Codman Square Health and Education Center capital expansion project, a $14.1M addition to the 637 Washington St. facility. Prior to joining Codman, he served as Chief Governance Officer at the Boston Stock Exchange, where his responsibilities included external relations, communications, governance and membership. An attorney by training, Tony serves or has served on a number of non-profit and foundation boards, including the Massachusetts Bar Foundation (past-president), Boston Securities Traders Foundation, Holy Family Hospital of Methuen (Caritas), Andover Retirement Board, and the Women’s Educational and Industrial Union, among others. Tony is a graduate of Boston College (’84) and Suffolk University Law School (’87).

Karen Van Unen is the Chief Operating Officer of DotWell (an affiliate organization that manages the shared resources of Codman Square Health Center and Dorchester House) and oversees the collaborative clinical programming of the two health centers, in addition to managing DotWell’s own community-based programs. Previously, Karen was with Boston Children’s Hospital.
Gabriel Vonleh

Gabriel Vonleh is a seasoned healthcare executive with over 22 years experience in operations management. Prior to joining Codman Square Health Center, Mr. Vonleh most recently served as Chief Operation Officer for MassHealth; where he was instrumental in the implementation of the electronic Document Management system (EDM). He also served as Vice President of Operations for Fund Administration and Membership Services at Amalgamated Life in New York; where he oversaw eligibility, collections, membership and external audits of participating funds. Before joining Amalgamated Life, Mr. Vonleh worked for Perot Systems Corporation as Director of Enrollment and Premium Billing for Blue Cross of Rhode Island. In that role, he managed operations, customer service and billing collection, among other responsibilities.
Mr. Vonleh is actively involved in providing mentorship to underserved youth in Massachusetts via the International Youth Partnerships; an organization he founded to encourage community volunteerism and leadership opportunities for young people. While in New York, Mr. Vonleh started a mentorship program for inner–city youth in Brooklyn at the Williamsburg High School. He resides in Sharon, Massachusetts with his wife and children.
He earned a Bachelor of Arts in business management/political science from Salem State University and a Master of Arts in Healthcare Administration from Suffolk University in Boston.

Candice has been at CSHC for seventeen years, and had been Chief of Staff for the past six years. She recently moved into her newest role as Public Relations Director and oversees Codman’s signature fundraising event, Men of Boston Cook for Women’s Health. She holds a BA from Loyola College in Baltimore, a certificate in Health Administration from Suffolk University, and an MPA from Suffolk. Candice serves on the boards of the Codman Square Neighborhood Council and the Dorchester Arts Collaborative. She is also Chair of the Codman Square Stakeholders Committee.

Juanita, a Dorchester native, is the Director of Human Resources. She built her career in Human Resources working exclusively with non-profits, including the Red Cross and Oxfam.

Liz MacNeil was president of the civic association that gave rise the Health Center, and she first worked at CSHC from 1982-1984 as Office Manager. Since returning in 1994 in the position of Manager of Special Projects, she has managed all construction and renovation projects. Liz holds a Bachelor’s degree and a certificate in Real Estate, and is a former real estate agent.

Jan joined Codman Square Health Center in May 2007. Previously, she was Director of Nursing at St. Elizabeth’s Medical Center of Boston. She is a registered nurse and has earned a BSN, a MS in Healthcare Management, and is certified in Nursing Administration.

Stephen Weymouth - President
A Dorchester resident, Stephen has served on the board since 1992. He is a criminal defense attorney who has been practicing law in Massachusetts for over 25 years. He holds a BA from Miami University and a JD from Suffolk University School of Law.

Emmett Schmarsow - Vice President
A resident of Milton, Emmett has served on the board since 1987. He has worked for over three decades as the Program Manager for Councils on Aging and Senior Centers for the Massachusetts Executive Office of Elder Affairs. There, he oversees state grants to 349 municipalities to provide support for elders (60+) to remain independent and at-home for as long as possible. Funding is used for a wide variety of direct and indirect support ranging from outreach, transportation and health screening to fitness and education classes, support groups, newsletters and volunteerism. Emmett is a recipient of the Manuel Carballo Award for Excellence in Public Service, the state’s highest honor for public employees. His other board affiliations include: Milton Fuller Village, ODWIN Learning Center, elected (Milton) town meeting member, MA Architectural Access Board (former) and MBTA Advisory Committee for Elders and Persons with Disabilities. Emmett has a BS from UMass, Amherst and a M.S. from RPI. In a semester away from college, he trekked solo in bicycle tour around seven European countries. He is a self-proclaimed ‘veggi-vore’ (mostly), and his hobbies include competitive racewalking, amateur photography and volunteering.

A resident of Chestnut Hill, Susan has served on the board since 1998. She works as an independent financial services consultant.
Thabiti Brown, Clerk
Thabiti has served on the CSHC Board since 2005. In his day job, he is the Principal of Codman Academy Charter Public School where he has worked since the school’s inception in 2001. He holds a BA in Urban Studies / American Civilizations from Brown University and a MA in Social Studies from Columbia University Teachers College. He has taught at the Beacon School in New York and at the International School of Panama, and is a recipient of the 2005 Educator Award from the Milken Family Foundation. Thabiti also serves on the Boards of the Civic Education Project (2004-present) and the Cambridgeport Children’s Center (2009-present). His hobbies include reading, writing, ‘rithmetic, basketball, and travel.

At Codman Square, signs of health include meaningful relationships between community members/organizations, access to quality education, and the development of new programming to meet the needs of members of the community. Egobudike (”Ego”) E. Ezedi, Jr.
A Dorchester resident, Ego Ezedi has served on the board since 2003. He is Nigerian-American, born and raised in Boston, Massachusetts. His experiences as a spiritual, political and business leader have given him a unique sphere of influence throughout the Greater Boston area. Egobudike formerly worked with U.S. Congressman Michael E. Capuano of the Eighth District in Massachusetts managing the Boston (main) office. Egobudike has been a candidate for political office, a host of the Health Matters radio broadcast, and Executive Director of the Roxbury YMCA. After experiencing the traumatic loss of his mother’s death 2005 to domestic violence, Egobudike has become an even stronger advocate for victims of domestic violence leading him to make it one of his life’s missions to stand against the evil of senseless violence everywhere. Currently, Egobudike is serving as the Sr. Pastor of Empowerment Christian Church, a ministry located in Dorchester. Egobudike is enrolled at Gordon-Conwell Theological Seminary as a degree candidate in the Masters of Divinity program and shares dual concentrations from Brandeis University and UMass Boston in sociology and psychology. Egobudike has been happily married to Melissa A. Ezedi for over 14 years and they have one beautiful daughter, Trinity Ekene Ezedi.
With all that said, Egobudike simply wants to be known as a man after God’s own heart.
Robert J. MacEachern
Robert is a third generation Codman Square resident, has been on the Board since 2003. Robert first became active in the Codman Square community while in middle school and his commitment to Dorchester hasn’t stopped. He was a member of the Mellen Street Crime watch, resurrected the long dormant Ashmont Hill Association and was elected its President during his junior year of high school. Robert also sits on the DotWell Board of Directors and served as Chairperson of that Board for two and a half years. Robert is a proud graduate of the Boston Public Schools with a learning disability. He has BA in Black Studies and Urban Studies from UMass/Boston. He has spent the past 15 years working with “at risk teens”. First with the UMass Boston Upward Bound TRIo Program, and most recently as the Senior counselor at the Home for Little Wanders Waltham House, which is a group home for GLBTQ youth. Robert counts one of the biggest blessings of his life, has been growing up and living in such a great and dynamic community.
Marva Serotkin
Marva has been on the board of CSHC since 1983. She is the President and Chief Executive Officer of The Boston Home, where she has worked for eleven years. Marva is the President of the Massachusetts Extended Care Federation and past President of the Massachusetts Public Health Association. She was recently named President of the Massachusetts Long Term Care Foundation. Additionally, she is an Assistant Clinical Professor at Tufts University and a lecturer for the Boston University School of Management, as well as a member of the Newton Health Commission.She holds a B.A. from Boston University and an M.P.H. from Yale University. Marva enjoys playing tennis.

Since I have been on the Board for so many years, I have witnessed the expansion of the scope of services that encompasses so many aspects of enhancing quality of life for all those who in the Codman Square area. CSHC is a model for integrating health care with the life of the community. The staff is dedicated, innovative and committed.”
Mona Scantlebury
Mona is a resident of Dorchester. She was a founding board member of Codman Square Health Center and has served on the board since 1976. She is a long-time community activist, with a 50-year history of service to the community. Currently, she is active in social service activities through the Union United Method Church.
A past Board Chair, Seaborn has been on the board for over 25 years since 1982, and is now an Emeritus Board Member. He is a retired Department of Defense manager who worked for the Defense Logistics Agency implementing the OSHA Act for military services in New England and New York from 1972 through 2003. In his retirement, Seaborn is a Docent at the Peabody Essex Museum in Salem. He was previously the Vice President of the Boston Branch of the NAACP Board, Chairman of the Board of the Boston Federal Safety Council, and a member of board of The Presidents Committee for the Employment of Persons with a Disability (appointed by President George H.W. Bush). Seaborn has been a member of the Museum of Fine Arts since 1980, a member of Toastmasters International since 1990 and a member of The Medford Historical Society since 2009. Seaborn has a BA in Liberal Studies in Sociology from Boston University and a M.Ed. from Antioch University. He has received numerous awards including the Superior Civilian Service Award and the Meritorious Civilian Service Award from the Defense Logistics Agency.

Charles O’Hara
Charles has been a long-time resident of the Ashmont Hill neighborhood of Dorchester from 1935 to the present. He is a founding board member of CSHC and has served since 1976. He is a past President of the Board, and Board representative to the Performance Improvement Committee. Charlie is a former teacher and he and his wife Seater were previously involved in Dorchester real estate. He holds a B.S. and a M.Ed. from Boston University and completed graduate studies at the Boston Teacher’s College. Charlie’s favorite food is pizza, and he enjoys camping, biking, swimming and hiking.
In his own words: “My favorite parts of Codman Square, Dorchester are the Great Hall, the White Church, and the High School.”
Ardis Vaughan
A Dorchester resident, Ardis has served on the board since 1998. She works as Project Director for Earth Tech, a global provider of consulting, engineering and construction services.
Rev. Warden joined the Codman Square Health Center board in December 2010. He is the pastor of the Greenwood Memorial United Methodist Church in Dorchester.Reverend Warden was born in Barbados, West Indies, spending much of his youth and young adult life there. Rev. Warden migrated to the United States in 1984, becoming a citizen in 1995. He earned a Master’s of Divinity degree from The School of Theology at Boston University, and continued his schooling by participating in a post master’s certificate program in Urban Ministry at the Urban Theology Institute in England.
He has served as Pastor at several other churches before coming to Dorchester. He also serves on the Board of Directors of the Four Corners Action Coalition, United Methodist Federal Credit Union, and the Greenwood Shalom Community Outreach. The Reverend enjoys cooking, reading, music and dancing.
His favorite part about Codman Square or Codman Square Health Center: “The willingness to serve those living on the margin”.
Sandra Warren
Sandra is a retired Director of Human Resources at Codman Square Health Center where she served for 7 years. She also worked as Director of HR for Trustees of Health & Hospitals/ City of Boston Department of Health & Hospitals. During that time, she worked on the closing of Boston City Hospital and facilitated its merger with University Hospital, which created Boston Medical Center. Sandra volunteers at the Brookview House, Dorchester, Mass. She holds a BA from UMass- Boston, and her hobbies include traveling, solving puzzles, and playing piano.

Bill Walczak
Co-founder and former CEO
Bill is most associated with the Codman Square Health Center, a multi-service center which he co founded in the 1970s. He served as CEO from 1979 through February 2011.
Bill is also founding president of Codman Academy Charter School, a high school located on the health center campus. In addition, he is a co-founder and former Co-CEO of DotWell, a partnership with the Dorchester House Multi-Service Center, which merged management and services to save money, build capacity and make a deeper impact on the community. This effort is considered a model of a nonprofit collaboration which uses the medical system as a platform for community regeneration.
